Mail In Registration Form - Please note if you chose to print and mail in your form and you are paying by credit card, your CVS code is required. For security reasons, we will not request this code on the form, but will input your registration into the system and select "pay in person". We will then email you instructions to the address listed on your registration for logging in to your account and completing your payment - only after you have made your payment will the registration be considered complete. If you plan to pay by credit card, please use the online registration if at all possible.
